چالش‌های انجام تریاژ صحیح: یک مطالعه مروری نظام‌مند

چکیده

زمینه و هدف: انجام تریاژ صحیح بیماران جهت دریافت خدمات درمانی می‌تواند باعث مدیریت ارجاع آن­‌ها به مراکز درمانی، کاهش میزان مرگ­‌و­میر و افزایش کیفیت خدمات شود. با توجه به اهمیت مسئله در این مطالعه به بررسی و توصیف برخی از چالش‌های موجود در زمینه تریاژ پرداخته شد.
روش‌ها: در مطالعه مروری نظام‌مند حاضر جهت استخراج مطالعات مرتبط، در پایگاه‌های اطلاعاتی داخلی ‌و ‌خارجی شامل Pubmed ،Scopus ،Science Direct ،SID و Magiran با کلید واژه‌های تریاژ، چالش، اورژانس‌ پیش‌بیمارستانی و اورژانس‌ بیمارستان از سال 1390 الی شهریور 1400 با هر دو زبان فارسی و انگلیسی براساس راهبرد جستجو انجام گردید. 20 مقاله براساس معیار‌های ورود وارد مطالعه شدند. 
یافته‌ها: از مجموع 6080 مقاله بازیابی شده، 20 مقاله بررسی گردیدند. در تحلیل و بررسی مقالات 7 مقاله در زمینه چالش‌های انجام تریاژ در سراسر دنیا شامل: 1. بروز خطای تریاژ و هدر رفت منابع ارزشمند اورژانس؛ 2. دانش ناکافی در رابطه با انواع الگوریتم‌های تریاژ‌ موجود؛ 3. تعدد الگوریتم‌های تریاژ، ایجاد الگوریتم‌های جدید و ناآشنا بودن آن‌ها برای پرسنل خدمات بهداشتی درمانی؛ 4. بروز معضلات اخلاقی برای تریاژکنندگان؛ 5. نبود ابزار‌های رایانه‌ای و هوش مصنوعی یا نا‌کار‌آمد بودن آن‌ها؛ 6. فاصله میان آگاهی و عمل پرسنل خدمات درمانی؛ 7. نبود دوره‌های تمرین شبیه‌سازی شده مدون، به‌دست آمد.
نتیجه‌گیری: چالش‌های متعددی در زمینه انجام تریاژ در اورژانس پیش‌بیمارستانی و بیمارستانی وجود دارد. با توجه به اثرات مخربی که این چالش­‌ها بر ارائه خدمات به بیماران می­‌گذارد، ضروری است تا اقدامات مقتضی برای رفع آن‌­ها از طرف تصمیم‌­گیرندگان به عمل آورده شود.
